An optional Complex Oral Surgery Rider will be available for the Dental PPO and TRP product offerings with an August 1, 2011 quote, September 1, 2011 effective date in the following states: IL, IN, ME, MI, MN, OH, TX and WI.
Basic Plan Designs:
- Move coverage for complex oral surgeries such as: surgical removal of erupted or impacted teeth, bone smoothing, removal of benign oral cysts or tumors; bone, cartilage, or synthetic grafts and trimming or removing over growth or non-vital tissue or bone; from major to basic coverage
- The option will be available on all PPO and Traditional Preferred (including PEM/PEB, INFS, MAF and Extended Max)
- Rate increase of one percent
- Case size: 2+
- 2-9 groups will continue to have waiting periods on the complex oral surgery codes.
