An optional Rider for Complex Oral Surgery will be available for quoting Feb. 1, 2011, with a March 1, 2011 effective date for the following states: DC, DE, NC, PA, SC and VA.

The complex oral surgery rider will increase our sales and provide less administration cost, overall decreasing custom requests, and reducing our custom plan summary requests.

Basic Plan Designs:

  • Move coverage for complex oral surgeries such as: surgical removal of erupted or impacted teeth, bone smoothing, removal of benign oral cysts or tumors; bone, cartilage, or synthetic grafts and trimming or removing over growth or non-vital tissue or bone; from major to basic coverage
  • The option will be available on all PPO and Traditional Preferred (including PEM/PEB, INFS, MAF and Extended Max)
  • Rate increase of one percent
  • Case size: 2+
  • 2-9 groups will continue to have waiting periods on the complex oral surgery codes.

The Employer group applications for the states of DC and PA will reflect the "Oral Surgery covered in Basic", with a "yes/no" option to select.

  • By selecting "Yes," the Complex Oral Surgery benefit will be included in the Basic with a one percent rate increase.
  • By selecting "No," the Employer is selecting to not include the Complex Oral Surgery benefit in the Basic.
  • If there is not a selection made on the Employer group application for DC and PA, the Complex Oral Surgery benefit will automatically default to Major, which is the base product.
  • The DC and PA applications will be updated and in production as of Jan. 22, 2011.

For the states of DE, NC, SC, and VA, sales will need to write-in the complex oral surgery rider on the Employer group application, if the group elects it. Otherwise, complex oral surgery will automatically default to Major, which the base product.

Directions will be communicated to sales associates in the affected markets and will be discussed on the February large and small group sales calls.
