Reforming healthcare requires rethinking the way chronic illness is managed. It demands a shift from a focus on sickness care to a whole-person approach to care management. On April 17, Humana is pleased to present Dr. David Nash, a member of the Humana Board of Directors and Founding Dean of Jefferson School of Population Health (JSPH), Thomas Jefferson University. Each year JSPH hosts the Population Health and Care Coordination Colloquium, which brings together healthcare professionals and academics to participate in a dynamic forum addressing challenges to wellness, prevention and care coordination programs. This year's colloquium, scheduled for Feb. 27-29, features topics including chronic illness management, health promotion, care delivery models, and consumerism.
