Humana is making it easier for Life/AD&D, Disability and Workplace Voluntary Benefits customers to submit enrollment forms, change forms, and applications.
Feedback from employers and agents helped to shape the new process, which includes instituting a new mailing address and offering a new fax option for submission of the forms.
The new address and fax number went into effect Nov. 1 and will need to be used to submit enrollment and service requests:
