Health Maintenance Organization (HMO)

With a Health Maintenance Organization (HMO) plan, covered members choose a primary care physician (PCP) from Humana’s network to coordinate all of their care.

How a HMO Plan Works

HMO plans give members access to care from a broad network of participating providers. Each family member chooses a PCP, builds a relationship with the PCP, and seeks the PCP’s guidance when specialty care is needed.

  • Referrals to specialists may be required

  • Members make copayments for most services

  • Coverage for services from out-of-network providers only in emergencies or when Humana has given prior authorization
