State Continuation

State Continuation is an employer/employee relationship for employers with fewer than 20 employees.

Most states have State Continuation laws that require employers to offer former employees and their dependents continuation of health coverage when they no longer are active under the employer's health plan. States may have different eligibility requirements that employees will need to meet.

Common State Continuation Questions

Where does the employee submit the State Continuation application?

The employee will submit the State Continuation application to the employer. This notifies the employer that the employee elected State Continuation, enables the group to review eligibility, and allows for premium arrangements if required by the state.

How does the employee and employer know if they qualify for State Continuation?

After Humana processes the State Continuation application, we'll send a confirmation letter to the employee and employer - or let them know that the employee does not meet state eligibility guidelines.

Where should the employer send the employee State Continuation application?

The employer can send the application to Humana by fax to 502-508-9978, by e-mail to Continuation@humana.com (please note this is an unsecured e-mail address), or by mail to:
