Scheduling a Consultation

Not all members are asked to schedule a consultation – only those with a higher risk level for harmful drug reactions.

If a member a notice in their SmartSummary RxSM statement to call MTM, we encourage them to do so..

Consultations are provided at no additional cost.

Scheduling

Your consultation will be with a registered nurse or a pharmacist. Consultations are conducted either over the phone or face-to-face. Please have these things ready when you call:

  • Humana ID number

  • SmartSummary Rx or a list of medications

  • A pen and paper for taking notes

  • Doctors’ addresses and phone numbers, in case we need to contact them about the consultation

Your consultation

In the course of a year, an MTM member can expect to receive one initial telephone or face-to-face consultation – which last up to 60 minutes – and follow-up consultations lasting up to 30 minutes each. As part of the consultation, the nurse or pharmacist will:

  • Answer any questions you have about the members' medications

  • Evaluate how and when to take the medications

  • Discuss any areas of concern about the medications or offer lower-cost drug alternatives

  • Contact your doctor(s) with recommendations after the consultation
