The agreement extends Humana's contractual relationship with Hinsdale Physicians Health Care, which has been in place since 1996.  It applies only to Humana's commercial HMO health insurance plans and does not include the company's Medicare Advantage health plans.  In Illinois, more than 80,000 health plan members access Humana's commercial HMO networks of health care providers.

Hinsdale Physicians Healthcare was founded in 1994 by a core group of physicians from Hinsdale Hospital to provide a health care delivery system for managed care patients.  In 1998, the physicians of Glen Care Managed Care joined the HPH network to expand the service area in the Western Suburbs to encompass areas around Glen Oaks Hospital and in 2003, the physicians of LaGrange Health Services followed suit.  This created a dynamic physician network with more than 400 Physicians who have access to three hospitals, a free-standing Emergency Care Center and Surgical Center.
