Track your family's immunization history

history
Most schools require students to provide proof of immunizations, along with a medical history to refer to in case of an emergency.

  • Talk to your doctor about the meningococcal vaccine.

  • Make sure your child has all the immunizations the school requires.

  • Create a health history folder for your children as well as for yourself and spouse.

Action Items
Ask about what vaccines are needed each time your child visits the doctor.

Be your child's health advocate by knowing what immunizations the American Academy of Pediatrics and/or what the Centers for Disease Control & Prevention (CDC) recommend at your child's age.

Visit www.aap.org or www.cdc.gov and search for "immunization schedule" to see the most current recommendations.
