According to the Centers for Disease Control (CDC), more than 20.8 million people in the U.S. have diabetes. One out of 10 healthcare dollars is spent on diabetes. In addition, the disease is associated with higher rates of lost work time, disability, and premature death. |
Program Goals
Humana's program is delivered by registered nurses who receive additional training in diabetes disease management. The goal is to slow the progression of the disease.
Services provided
The program is designed for adults with both type 1 and type 2 diabetes. Participants receive guidance about:
- Medication management and drug adherence
- Diet and exercise
- The importance of monitoring
The bottom line
Past program results show participants are:
- 25 percent above benchmark for having an annual eye exam
- 60 percent above benchmark for having an annual foot exam
