Personal Nurse®

Personal Nurse helps members take charge of their health and fit healthy choices into their lives. Specially-trained nurses call members who may be at risk for, or currently dealing with, serious conditions like diabetes or high blood pressure.

The Role of the Personal Nurse

Although Personal Nurse is a phone-based service, it’s different from a nurse-on-call hotline. Participants work one-on-one with a nurse who helps them understand their condition, find resources, and use their plan benefits effectively – all in an effort to help them realize their health goals.

Services provided

Participants develop a long-term, trusted relationship with their Personal Nurse. The nurse:

  • Encourages the member to be actively involved in healthcare decision-making

  • Builds a supportive relationship

  • Offers personalized education and guidance

  • Helps the member use his or her benefits effectively

  • Works with the member to identify roadblocks to improved health

The bottom line
