Humana's traditional medical plans

  • Preferred Provider Organization (PPO) plans offer a wide choice of providers, with discounts for using in-network doctors and hospitals. These plans are a popular option because they give members freedom of choice, with the opportunity to reduce their costs. Members can choose any provider – but they pay less for care when they go to doctors, hospitals, and other healthcare providers in Humana's extensive network.
  • Health Maintenance Organization (HMO) plans offer predictable out-of-pocket costs and emphasize the relationship with a primary care physician. Referrals to specialists may be required.
  • National and Regional Point of Service (POS) plans combine the cost savings of our regional HMO networks and the flexibility of a PPO. Members can stay within the network and pay a lower share of the cost – or they can seek services outside the network and pay more. In addition to traditional POS plan designs, we offer consumer-driven POS plans.

Humana's network offerings for traditional plans vary by area. Your options may include:

  • Humana/ChoiceCare ® Network is our nationwide PPO network, with more than 550,000 unique providers and approximately 3,500 hospitals; for more details, visit ChoiceCare Network
  • National POS – Open Access offers improved discounts and a network of providers that overlaps 95 percent with the national Humana/ChoiceCare Network
  • Humana Preferred SM is a series of PPO and POS plan offerings, supported by a high-performance network

We also offer Classic plans, which provide indemnity-style coverage for members who live outside Humana's network areas.
