- Eye exam – The plan provides a comprehensive eye health exam for a fixed copayment
- Frames – Members receive an allowance toward frames; an additional discount is applied after the allowance
- Lenses – Members have a $0 copayment for lenses
- Lens options – Members have set copayments for lens options such as anti-reflective and UV coatings
- Contact lenses – An allowance is available for contacts lenses in lieu of eyeglass lenses; members can get substantial savings by ordering replacement contact lenses online
- Laser procedures – Members have access to discounts on LASIK procedures from participating independent laser centers
Focus
The Focus plan helps members save on eyewear expenses by offering an annual eye exam with a $10 copayment, plus benefits for lenses, contact lenses, and frames. In-network and out-of-network benefits are available.
