Q. How do employees cancel workplace voluntary coverage?
Employees who want to cancel their coverage should fill out and sign our Voluntary Benefits Cancellation Request Form (Form 1618). A copy goes to the employer and the original goes to Humana Specialty Benefits. Humana Specialty Benefits must receive written notice to cancel coverage.
