What is MTM?

Medication Therapy Management (MTM) is a federally designed program sponsored by Humana that helps educate Medicare members about their medications.

MTM will help you:

• Get the greatest benefit from your medications
• Reduce the risk of harmful side effects
• Find lower-cost alternatives to your prescribed medications

Eligibility and Services There are minimum requirements for participation in MTM. When these requirements are met, members receive MTM information in their next SmartSummary RxSM.

Scheduling a Consultation Members who meet eligibility requirements may be offered the opportunity to schedule a telephone or face-to-face consultation.

Patient Safety and Follow-up The MTM program is federally designed because it's important for good health. If members are asked to participate, we encourage them to do so.
