Overview

The Classic plan - also known as a "traditional indemnity plan" or a "fee-for-service plan" - has no provider network. You can choose any doctor or other healthcare provider, and you don't need a referral to see a specialist.

How it works

With a Classic plan, your costs are the same for any provider - you aren't required to use in-network doctors.

Your out-of-pocket costs may include copayments, deductible, or coinsurance.

How It Works

A Classic plan is a good choice if you are willing to pay more for coverage to have the same level of benefits with any provider.

Here's how it works:

  • Your costs are the same for any provider. You don't have to worry about paying more for certain doctors and hospitals.
  • You can keep the same doctor. This plan doesn't require you to see in-network doctors.
  • You're covered wherever you go. This plan offers flexibility for people who travel or live in an area with few in-network providers.
  • Your out-of-pocket costs may include copayments, deductible, or coinsurance.
  • If you reach the out-of-pocket maximum, the plan pays 100 percent of additional covered expenses during the plan year. You continue to pay copayments.
