How it works

Humana Simplicity is different from other medical plans. When you use your plan for in-network healthcare services, you pay a copayment for that service – there's no deductible. This straightforward plan design makes it easy for you to understand your benefits and payment responsibilities.

HumanaVitality℠ is also included with the plan. It can help you take control of your health and make healthy choices. Tailored to each person's unique health status and lifestyle, it's designed to be fun and easy, no matter what your level of fitness.

How It Works

Humana Simplicity is a medical plan that's easy to use, easy to afford and easy to understand.

When you use your plan for in-network healthcare services, you pay a copayment for that service – there's no deductible. This straightforward plan design makes it easy for you to understand your benefits and payment responsibilities.

Key Benefits:

  • Robust plans, and various networks options.
  • No in-network deductibles or coinsurance.
  • All copayments count toward maximum out-of-pocket expenses. Once the out-of-pocket- maximum has been reached, the plan will pay 100 percent of the eligible charges under the written terms of the policy.
  • Preventive care services are covered at 100 percent for in-network providers.
  • Care from any provider in the network without a referral.*
  • Access to urgent care facilities, including Concentra®, for a lower copayment than the emergency room.**
  • Choice of pharmacy plan options and copayments.

Humana's network offerings for Humana Simplicity vary by area. Your options may include:

  • Humana/ChoiceCare®
    • Network is our nationwide PPO network, with more than 550,000 unique providers and approximately 3,500 hospitals; for more details, visit ChoiceCare Network.
  • National POS
    • Open Access offers improved discounts and a network of providers that overlaps 95 percent with the national Humana/ChoiceCare Network.
  • HMO Premier
    • The largest HMO network available for our members.

* Our health benefit plans have limitations and exclusions

Humana's mobile resources are designed to support the on-the-go needs of consumers and empower healthcare decisions at the point of service. Use your existing MyHumana login to access the resources on your mobile device's browser.

Humana Commercial Preauthorization and Notification Lists

These dated documents provide a complete list of services that require preauthorization or prior authorization. Some employer groups for which Humana provides administrative services only (self-insured and employer-sponsored programs) may customize their plans with different requirements.
