Your plan may include an over-the-counter (OTC) monthly credit. Please call a Customer Care specialist to verify your eligibility, determine your benefit amount, or request an OTC order form at 1-800-457-4708. If you use a TTY, call 711, seven days a week, 8 a.m. – 8 p.m.
For details on how Medicare members can get discounts on over-the-counter (OTC) medications, download the order form below. Please refer to your Summary of Benefits or Evidence of Coverage (EOC) for details. If you're eligible for a monthly OTC credit, deduct that amount from the "You pay" line on the order form.
