The PFFS plan is very simple. The plan doesn't have a list of providers, so members can choose any medical provider who participates with Medicare and agrees to accept Humana's payment terms and conditions.

You also have access to a variety of programs and services that promote healthy living and aging – at no additional cost. Value-added features include fitness programs and Humana Active Outlook^SM.

How the plan works

With the Humana Group Medicare PFFS plan, you enjoy several features and benefits:

• Freedom to choose doctors, specialists, and hospitals
• Doctor’s office visits and annual routine physicals
• No copayment for preventive services, including cancer screenings and immunizations, for most plans
• Emergency coverage anywhere in the world

Some Humana Group Medicare PFFS Plans include prescription drug coverage, which is equal to or better than the minimum requirements for a Medicare Part D plan.
