Doctor-patient relationship is the key
With the HMO plan, you work one-on-one with a primary care physician (PCP) who coordinates your care and provides referrals to other in-network providers when necessary.* Plus, you aren't locked in once you choose a PCP – you can switch to another in-network doctor who is accepting new patients anytime.
Humana Gold Plus includes all the benefits of Original Medicare, plus prescription drug coverage and added features like Humana Active OutlookSM. The SilverSneakers® Fitness Program is also available with many of our Medicare HMO plans.
