The bariatric hospital and the surgeon must be designated a Center of Excellence to be in-network for bariatric surgery benefits.
In-network certification
Both surgeons and hospitals in Humana's bariatric surgery network must be certified as a Center of Excellence by one of the following:
- American Society of Metabolic and Bariatric Surgery/Surgical Review Corporation
- American College of Surgeons' Bariatric Surgery Center Network

The hospital and the surgeon must also participate in your health plan for you to get the highest level of coverage
