Caregiver Access
Many people trust someone else to help with healthcare matters. This person may be a grown child, spouse, sibling, or close friend. Humana calls this person a "caregiver." We can share your information with a caregiver – but, by law, we need your permission first.
Reasons to have a Caregiver
A caregiver can help you:
- Talk to Humana about your insurance plan
- Keep track of your benefits and claims
- Interpret healthcare information
- Get the care you need in an emergency
How to Authorize a Caregiver to Interact on Your Behalf
To give your permission, you'll need to read and sign a consent form. Here's what to do:
- Download and print the document below
- Fill out the "Consent for Release of Protected Health Information" page
- Sign the form
- Follow the instructions on the form to mail or fax it back to Humana
This consent only allows us to share information. It's different from granting medical power of attorney – which allows someone to make decisions about your care.
Another option is to give us authorization over the phone. With this option, you must be present to give your authorization every time your caregiver talks with Humana on your behalf.
