1. Humana Preferred (BHCG) Option - This option is available to employees living in southeast Wisconsin (Kenosha, Milwaukee, Ozaukee, Racine, Walworth, Washington, and Waukesha counties).

2.Humana/ChoiceCare Broad Option - This option is available to employees living outside southeast Wisconsin (excludes Kenosha, Milwaukee, Ozaukee, Racine, Walworth, Washington, and Waukesha counties). This option utilizes the HUMANA/CHOICECARE NETWORK PPO which includes providers nationwide, including Wisconsin.

Once you find a provider, be sure to click on the provider’s name in order to verify the location(s) that is considered in-network. It is possible that providers practice with multiple groups and that all locations may not be in the network. Under the "Plans Accepted" heading, be sure to look for Humana Preferred (BHCG) , BHCG Out-of-HP Service Area , or Humana/ChoiceCare Network PPO  if you are searching for providers in the Humana Preferred Option; or Humana/ChoiceCare Network PPO if you are searching for providers in the Humana/ChoiceCare Broad Option. If these network names are not listed, the provider’s location is not considered in-network under the plan. In addition, be sure to verify in-network hospitals. Even if your provider is considered in-network, he/she must admit to an in-network hospital in order to receive in-network benefits for hospital expenses.
