Fee Schedules The Fee Schedule Inquiry function allows you and your office staff to review specific fee schedules that are applicable to your provider agreements with Humana. After you enter your Provider name and Tax Identification number, a list of fee schedule/s applicable to your individual contract will be displayed along with a description of the schedule and the area. After you select a fee schedule, you will be able to enter a list of up to twelve CPT4 or HCPCS codes for fee information. You may choose to enter a range of codes or a specific list. The code information will be returned for you to view.
