Here’s how we help members better understand their medications.
About Medication Therapy Managment (MTM)
Medication Therapy Management (MTM) is a federally designed program sponsored by Humana that helps Medicare members to:
- Know more – about getting the greatest benefit from their medications
- Reduce risk – by learning how to avoid harmful side effects
- Save money – by finding lower-cost alternatives to prescribed medications
All MTM members receive monthly mailings on topics like compliance, medication profiles, and questions to ask their pharmacist. Some are eligible for a consultation – face-to-face or by phone – with a health professional.
Member eligibility
Humana use members’ proprietary information to determine eligibility for the MTM program, like:
- Medications: The number of medications they take
- Medication costs: Anticipated drug costs of $4,000 per year or more
- Health condition: The number of diagnosed chronic illnesses they have
- Plan enrollment: Members must be enrolled in either a Part D Prescription Drug Plan or a Medicare Advantage plan with prescription drug benefits
Membership is for an entire calendar year and is re-evaluated annually.
