Delivers Injectables

Caremark® Specialty, a division of Caremark, is a full-service specialty pharmaceutical provider that will deliver injectable medications and other specialty medications to the homes of Humana members, physicians’ offices or other delivery points.

Caremark Specialty Pharmacy is Humana’s preferred provider of specialty injectables and is a valuable resource for physicians. It eliminates the need to stock high-cost injectables and streamlines the process for physicians who order injectables for Humana-insured patients. In addition, medications are delivered within 24 hours for orders received before 3 p.m. CST (provided that all information is on the referral form and prior authorization or other additional information is not necessary).

Call Caremark Specialty at (866) 295-2779 to submit prescriptions by phone; or referral forms can be faxed to Caremark Specialty at (866) 295-2778. Caremark will notify the physician if prior authorization is needed. The physician will need to call Humana to obtain prior authorization. The physician is not billed for the medications; Caremark Specialty collects copayments or coinsurance from the Humana member, and the balance is paid by Humana. The physician only needs to bill the patient for the office visit, not the medication. Caremark Specialty provides injectable medications for many chronic conditions including:

  • Multiple Sclerosis
  • Growth Hormone Deficiency or other FDA-approved indications
  • Psoriasis
  • Primary Pulmonary Hypertension
  • Hepatitis C
  • Rheumatoid Arthritis
  • Hemophilia
  • Gaucher’s Disease
  • Cystic Fibrosis

Caremark Specialty Frequently Asked Questions

Q. How many doses may be ordered at a time?
A. There is a limit of a 30-day supply for injectable medications; however, you may order refills up to one year in advance from Caremark Specialty. The Caremark pharmacy will contact the office or patient to coordinate future refills on medications.

Q. Who do I call if I order medications and they are not delivered?
A. Contact Caremark Specialty at (866) 295-2779, Monday through Friday 7 a.m. to 6 p.m. Central Time.

Q. How can I obtain additional referral forms?
A.
You may access the specialty injection medication patient referral form at www.humana.com. From the home page, do the following:

  • Click on the “Pharmacy” link on the left.
  • Click on “Tools and Resources Main Page” from the list on the left.
  • Select “Drug Lists and Forms”
  • Then choose “About SpecialtyRx and SpecialtyRx Forms.”
  • Click on “General Specialty Injection Medication Patient Referral Form” to find a printable version of the form.

You may also contact Humana Clinical Pharmacy Review at (800) 555-CLIN (800-555-2546) and a representative will fax the form to your office.

Q. Can I order any injectable through Caremark Specialty?
A.
Caremark Specialty can provide most injectables prescribed by a physician for Humana-insured patients, excluding Imitrex, insulin, Byetta, Smylin, Glucagon, Anakit, Epi-Pen, Depo-Provera or Depo-Testosterone. These injectable medications may need to be obtained from a local retail pharmacy.

Q. To whom can Caremark Specialty ship medication?
A.
Caremark Specialty can ship to the physician’s office, to the Humana-insured patient or to other designated delivery points. Typically, self-administered injectables are shipped directly to the patient and office-administered injectables are shipped to the physician’s office due to the proper handling needed for medications that are administered in the physician’s office.
