Program Overview

This disease management program for complex, chronic conditions is designed to help members with the management of the following 13 rare diseases:

  • Amyotrophic Lateral Sclerosis (Lou Gehrig's disease)
  • Chronic Inflammatory Demyelinating Polyradiculoneuropathy (CIDP)
  • Cystic Fibrosis
  • Dermatomyositis
  • Hemophilia
  • Lupus (Systemic Lupus Erythematosus)
  • Multiple Sclerosis
  • Myasthenia Gravis
  • Parkinson’s disease
  • Polymyositis
  • Rheumatoid Arthritis
  • Scleroderma
  • Sickle Cell Disease

The rare conditions program identifies and stratifies individuals with applicable diseases, using a claims-based algorithm to analyze procedure, drug and diagnosis codes from Humana data.

Program Components

  • Care coordination and patient education via telephone contact from Humana nurses
  • Continued communication with patients and their families, including anticipation and resolution of patients’ concerns, as well as general support
  • Phone access to specially trained nurses

The Humana disease management program for complex, chronic conditions is available to Medicare HMO and commercial HMO, PPO and POS members in all Humana markets, except Puerto Rico.
