Program Overview
Good nutrition is essential for your patients' optimum health, especially following a hospitalization or skilled nursing facility stay. Well Dine offers 10, frozen, precooked meals to eligible members after their discharge from a hospital or nursing facility – at no extra charge to the member.
How the Program Works
- After an overnight inpatient stay in a hospital or skilled nursing facility, the member may be eligible to receive 10 frozen, precooked meals – at no charge.
- The member may call 1-866-96MEALS (1-866-966-3257) to see if he or she is eligible. Well Dine representatives are available Monday through Friday from 8 a.m. to 9 p.m. Eastern time. If the member's Humana plan offers this program, he or she will be able to schedule delivery during the same call.
- The member will receive his or her meals with a guide on healthy eating.
- The member can choose from meal types, such as regular, diabetic and puree.

Encourage Participation
If you have Humana Medicare Advantage patients who are currently receiving care at a hospital or skilled nursing facility, we would like to request that you encourage them to check if they are eligible to take advantage of this no-cost service.
