Neonatal Intensive Care Unit (NICU) Program

Humana Case Managers along with a vended relationship will provide telephonic or onsite case management services for eligible infants admitted to a Neonatal Intensive Care Unit.

Program Overview

The NICU case management program works closely with Medical Directors, families and hospital staff to coordinate care during the infant's stay in the hospital and after discharge.

Program components

  • Case management services by experienced registered nurses
  • Coordination of care between health care providers
  • Planning for home care after hospital discharge
  • Discharge planning support to arrange home health or durable medical equipment as required
