Overview
This nine-month program is available to eligible Humana members with high-acuity coronary artery disease (CAD). Humana contracts with CorSolutions Inc. to coordinate care and augment the treatment plan prescribed by a member's physician. The program is supported by CorSolutions' disease-specific protocols and specially designed behavior-modification tools.
Program Components
- Proactive identification of candidates for the program through the use of medical data, physician referrals and precertification information.Registered nurses are available via telephone 24 hours a day, seven days a week to answer questions from the patient and the patient's family members.
- Close contact is maintained with the participating physician and the patient to help coordinate care, to provide educational materials and to improve communication.
