Overview
This disease management program for complex, chronic conditions is designed to help members with the management of the following 13 rare diseases.
•Amyotrophic Lateral Sclerosis (Lou Gehrig's Disease)
•Chronic Inflammatory Demyelinating Polyradiculoneuropathy (CIDP)
•Cystic Fibrosis
•Dermatomyositis
•Hemophilia
•Lupus (Systemic Lupus Erythematosus)
•Multiple Sclerosis
•Myasthenia Gravis
•Parkinson’s Disease
•Polymyositis
•Rheumatoid Arthritis
•Scleroderma
•Sickle Cell Disease
The program became operational in July 1998 through an agreement between Humana and Accordant Health Services, an independent disease management company that develops and operates integrated care-management systems for individuals with certain complex, chronic conditions.
Through this program, Accordant
•Identifies and stratifies individuals with applicable diseases, using patented algorithms to analyze procedure, drug and diagnosis codes from claims data.
•Proactively and routinely monitors patient status through a variety of communication channels, including the telephone and the Internet.
•Contributes to case-management activities to minimize the effect of crisis events.
Program Components
•Introduction of services to Humana-contracted physicians through various means, including forums and mailings.
•Identification of patients with rare diseases through the use of medical data, pharmacy claims data, referrals and the interfacing of Humana’s data recovery methods with those of Accordant Health Services.
•Care coordination and patient education via telephone contact from Accordant.
•Continued communication with patients and their families, including anticipation and resolution of patients’ concerns, as well as general support.
•24-hour, toll-free access to specially trained nurses.
•Access to Accordant’s Web site, through www.accordant.com, to provide live forums, chats, interactive tools, links to health care resources and other information.

The Humana disease management program for complex, chronic conditions is available to Medicare HMO and commercial HMO, PPO and POS members in all Humana markets except Puerto Rico.
