Overview
The HF program is a physician-directed, nurse-monitored program for Humana members with New York Heart Association Class III and IV heart failure. Class III and IV represents a member's level of functionality, with Class IV indicating the lowest level. Humana's contractor for the HF program is CorSolutions Medical Inc., a leading provider of integrated cardiac management services for high-risk patients. This initiative began several years ago, with a highly successful two-year program for Humana members with HF.

The HF program is based on the MULTIFIT SM system, which was developed at the Stanford University Department of Cardiac Rehabilitation and is licensed exclusively by CorSolutions Medical, Inc. It utilizes a team of trained nurses to coordinate care under the attending physician's direction and with the support of disease-specific protocols and specially designed education and behavior-modification tools. The nurses work with Humana members to address clinical and behavioral issues, such as medication compliance, dietary sodium restriction, exercise and early symptom identification. In addition, the nurses provide progress reports to the attending physician.
