Disease Management Program for Complex, Chronic Conditions
This program for complex, chronic conditions is designed to assist members with the management of 13 rare diseases.

Heart Failure Program

This physician-directed, nurse-monitored program is for Humana members with New York Heart Association Class III and IV congestive heart failure.

Coronary Artery Disease (CAD) Program

Eligible Humana members who have been diagnosed with coronary artery disease have access to this nine-month program, which coordinates care and augments the treatment plan described by a member's physician.

Renal Disease Management Program

This program is designed to provide support for patients with end-stage renal disease (ESRD).

Neonatal Intensive Care (NICU)

Nurses work closely with the families of infants who require neonatal intensive care, their physicians and hospital staff to help coordinate the infants' care.

HumanaBeginnings Program

This is a prenatal education and case-management program for pregnant members.

HumanaFirst

This 24-hour, seven-day-per-week nurse triage service guides members to the health care information and resources they need to select the most appropriate level of care.
