How to Register

By email:

To register, send an email to lwingo@humana.com. When registering, please include the words "Registration" and the date and time of the session in the subject line of the email (e.g., "Registration – 1/25/2012, 10 a.m.").

Also include the following information in the email:

• Name of each participant (indicate if you are requesting continuing education units)
• Practice name
• Phone number
• Fax number
• Email addresses of participants
• Tax ID number

By Fax:

If you are unable to email your registration, please fax the same information to 210-582-3690.

You will receive a confirmation and instructions for accessing the online seminar via email or return fax. Continuing education unit certificates (if applicable) will be individually provided after each session.
