Heart Failure (HF) Program
Humana's HF program is a physician-directed, nurse-monitored program for Humana members with New York Heart Association Class III and IV heart failure.
Class III and IV represents a member's level of functionality, with Class IV indicating the lowest level. |
Program Overview
Humana contracts with CorSolutions, a leading provider of integrated cardiac management services for high-risk patients, to administer its HF program.
How the program works
The heart failure program seeks to coordinate care and complement the treatment plan prescribed by a member's physician. The program is supported by CorSolutions' disease specific protocols and specially designed behavior modification tools.
- It utilizes a team of trained nurses to coordinate care under the attending physician's direction and with the support of disease-specific protocols
- It includes specially designed education and behavior-modification tools
- Nurses work with Humana members to address clinical and behavioral issues, such as medication compliance, dietary sodium restriction, exercise, and early symptom identification
- Nurses provide progress reports to the attending physician
Program components
- Continual screening of claims records to identify candidates for the program
- Linking eligible members with a CorSolutions registered nurse
- Ongoing education and monitoring in the areas of diet, exercise and lifestyle
- Working with members' primary care physicians (PCPs) to improve outcomes
More information
The HF program is available to Medicare HMO and commercial HMO, PPO, and POS members in all Humana markets except Puerto Rico. Members may self-refer to the program or be referred by their physician. CorSolutions then provides an assessment to determine whether the member meets the criteria for enrollment.
