Enhanced Benefits for
Humana Active Outlook in 2008

Humana Active Outlook^SM is a lifestyle enrichment program that provides Humana’s Medicare members with active living programs that address the physical, psychological and social aspects of healthy aging. The program is available at no additional cost to Medicare Advantage members.

Since the program’s introduction in 2006, Humana Active Outlook has been well received by Humana’s Medicare members. This year, Humana has added a new module called Heal!, which provides condition specific information on common health concerns and the management of chronic conditions. In addition, Humana Active Outlook now offers more educational opportunities for Medicare members.

Heal! offers information on:

• Diabetes
• Cancer
• Weight management
• Cardiovascular disease
• Back health and care
• Management of chronic conditions

The Heal! programs provide one-on-one interaction with a specially trained program nurse. Program nurses provide guidance on achieving individual health goals, help members better manage their conditions and avoid unnecessary progression of disease, and provide planning tools and resources that enable members to better manage their health conditions.

Cancer, diabetes and cardiovascular programs
The cancer, diabetes and cardiovascular programs are administered by Humana. Eligible members receive a welcome kit in the mail, which includes nurse contact information, a program overview, planning and educational materials specific to the member’s disease, and the Heal! newsletter related to the member’s disease. Program nurses also administer a prehabit and post-habit survey to evaluate behavioral changes as a result of the education materials.

Weight management, back health and chronic conditions programs
The “Balance” weight management program, the “Care for Your Back” back health and care program and the “Care for Your Health” chronic conditions program are administered by Corphealth. Members refer themselves to these programs by calling Corphealth at (866) 452-6442 or by visiting HumanaActiveOutlook.com. After entering their ID number, members can complete a prequalification screening to determine eligibility. Eligible members receive an educational guide on their specific program, customized to their needs. In addition, members receive several issues of program newsletters via the Web or mail, whichever they choose. The programs also include unlimited telephone or Web coaching, access to a dedicated Heal! section of HumanaActiveOutlook.com and local disease-specific classes.

Community classes
This year, Humana Active Outlook also offers an expanded selection of local community classes. Classes taught by specially trained Humana Active Outlook health educators are available in 75 markets in 90 locations. Class topics include the following:

• Nutrition As You Age
• Alternative Exercise (yoga, tai chi)
• Prescription Drug and Vitamin Interactions
• Technology 101
• Container Gardening
• Advance Care Planning
• Getting the Most From Your Benefits

Humana Encourages Use of
Mail-Order with RightSource^SM

Humana encourages members to take advantage of mail-order with RightSource, Humana’s prescription home-delivery service. Some of the advantages of using RightSource include the following:

• Opportunities to save members time and money
• Convenient ordering and delivery by mail
• Better medication compliance (i.e. three-month supply, refill reminders)

Physicians can call in a member’s prescription to RightSource by dialing (800) 379-0092 or by faxing prescriptions to RightSource at (800) 379-7617.
