New Names for Two Disease
Management Vendors
Two of Humana’s Disease Management program vendors have new names. These programs are available to members of commercial health plans and members of self-insured (ASO) plans that have elected the disease management option.
Please note that self-insured employer groups select options to best fit their employees and their families’ needs, and therefore not all self-insured plans include clinical programs.
ParadigmHealth is now Alere
Humana’s neonatal care management vendor, ParadigmHealth, has been acquired by Inverness Medical Innovations/Alere. Humana’s Neonatal Intensive Care Unit (NICU) program remains the same under the new organization, which has integrated ParadigmHealth’s services with Alere’s disease management programs.
Alere’s board certified neonatologists, neonatal nurse care managers and pediatric specialists work closely with members and their attending physicians and nurses to provide information and coordinate services for low birth-weight, medically complex and high-risk infants in an NICU and/or a specialty care unit. The program provides utilization review, case management and coordination of health care services for newborns from day of admission to an NICU through any required transitional and/or home care service up to 30 days post-discharge.

AirLogix is now Nurtur
AirLogix has completed a merger with Cardium Health, which blends the two companies’ services under the name Nurtur.
Humana’s Asthma Disease Management program remains the same under the new organization. The program’s focus is supporting members with adherence to medications, identifying and avoiding triggers and promoting use of an asthma action plan, peak flow meters and spacers.
- Eligibility: Commercial members (adults and children who are at least 2 years old) with unstable or erratic asthma control.
