Humana Offers Specialty Conditions Program
Humana will offer its providers and
members a new internal program called Specialty Conditions, through
Humana Cares, starting in January. Specialty Conditions will replace
Humana's Rare Diseases program from Accordant Health Services.
Specialty Conditions is available to fully-insured Humana members
and ASO groups whose company has purchased the program.
The new program from Humana Cares, Humana's
complex care management program, will take a holistic and proactive
approach to health care and provide support and customized coaching
for the member, his or her spouse, family and caregiver.
The conditions covered by the program
will be the same, and are:
- Amyotrophic lateral sclerosis (ALS)
- Chronic Inflammatory Demyelination Polyradiculoneuropathy
(CIDP)
- Cystic fibrosis
- Dermatomyositis
- Hemophilia
- Multiple sclerosis (MS)
- Myasthenia gravis
- Parkinson's disease
- Polymyositis
- Rheumatoid arthritis (RA)
- Scleroderma
- Sickle cell anemia
- Systemic lupus erythematosis
Enrolled members of the Rare Diseases
program will receive a letter about the transition, as well as
a telephone call from their current Accordant nurse sometime in
December. Humana will call transitioning members starting at the
beginning of the year.
Physicians are advised to talk with
their Humana-covered patients about the benefits of Specialty Conditions
and encourage them to work with Humana nurses.
