Humana's Medicare Advantage Health Maintenance Organization (HMO) plan gives members broad coverage and predictable out-of-pocket costs.

Doctor-patient relationship is the key
With the HMO plan, members work one-on-one with a primary care physician (PCP) who coordinates their care and provides referrals to other in-network providers when necessary. Members can switch to another in-network doctor anytime.
Members get all the benefits of Original Medicare, plus added features like a fitness program and Humana Active Outlook.
How the plan works
- Choice of a primary care physician in the plan network
- Low copayments for medically necessary doctor's visits and annual routine physicals
- Coverage for most annual screenings at no charge
- Emergency coverage when they travel outside the United States
- Low member cost-share
- Optional Humana prescription drug plan that gives members coverage equal to or better than standard Medicare Part D.
