1. From the Claim Type drop-down box, select either search option S3/4 Claims rejected S3/4 with no referral or PEND 20 - Claims pended with no referral.

All fields with a red asterisk (*) must be completed.

The Search Date will default to the current date or a date within 14 days prior to the current date may be entered.
