Humana Meals on Wheels

Good nutrition is essential for your patients’ optimum health, especially following a hospitalization or skilled nursing facility stay.  The Meals On Wheels Association of America Foundation (MOWAAF) and Humana have teamed up to offer 10 frozen, precooked meals to Humana Medicare health maintenance organization (HMO) and local preferred provider organization (LPPO) members in Arizona, Chicago, Central and South Florida, Corpus Christi, Kansas City and Louisiana after their discharge from a hospital or skilled nursing facility – at no additional charge to the member.

How the service works:

  1. Before discharge, the member will receive a get-well card from Humana, delivered by a Humana nurse or facility staff member.  The staff member will encourage the member to call 1-866-96MEALS (1-866-966-3257) to sign up, if the member is interested. 
  2. Once the member calls to sign up, he or she will be able to utilize Humana’s interactive voice response (IVR) system to enter the following information:  date of birth, Humana ID number, return phone number and anticipated discharge date. 
  3. A Meals On Wheels representative will call the member back to schedule a convenient time for delivery of the meals.
  4. The member can specify a regular meal, diabetic/low-sodium, kosher or pureed.  (Please note:  If the meal choices do not fit the physician’s orders for the member, the member should follow the physician’s orders and request the Meals On Wheels delivery after the restrictions are removed.)
  5. Once the member is discharged, Meals On Wheels will deliver 10 frozen meals to his or her home.

If you have Humana Medicare HMO- or LPPO-insured patients who are currently receiving care at a hospital or skilled nursing facility in the participating areas, we would like to request that you encourage them to take advantage of this no-cost service.
