Humana's Oncology Quality Management program, founded on evidence-based care, utilizes a counseling model within traditional preauthorization management for chemotherapeutic drugs, symptom management drugs and supporting agents. This program is administered by New Century Health for Humana commercial and Medicare Advantage (MA) members in Chicago, IL; Cincinnati, OH; Kentucky; and Milwaukee, WI. Please note that precertification and preauthorization refer to the same process of preauthorization. For MA Private Fee-for-Service (PFFS) plans, notification is requested, not required.

Submitting Preauthorization Requests

To request preauthorization, the requesting physician must complete and submit a chemotherapy treatment request (CTR) form.

Choose from the following options to submit the CTR form:

  • Initiate an online preauthorization request for an affected drug by logging on to New Century Health's website at www.newcenturyhealth.com. Choose "Provider Login" at the top of the screen. Enter your username and password or choose "Create an account" if this is your first visit.
  • Fax the completed CTR to New Century Health at 1-888-780-4925
  • Contact New Century Health’s intake coordinator department at 1-855-427-1372, option 5, 8 a.m. to 8 p.m. Eastern time, Monday through Friday.

Please note: Employer groups for which Humana provides administrative services only (self-insured, employer-sponsored programs) are excluded from this requirement.
