What are those abbreviations on your member ID card?

The card will list only the employee's name unless your state requires dependents to be listed or you're enrolled in a prepaid or DHMO plan.

Your dental member identification card provides valuable information to your dentist. In case you've been wondering, the abbreviations under "coverage" let your dentist know whom your plan covers.

Details

  • EE: Employee Only
  • ESP: Employee and Spouse
  • ECH: Employee and Child/Children
  • FAM: Family

You receive one card at your home address for Employee Only and Employee and Child/Children coverage, including dependents over age 18. Employee Only and Spouse, and Family coverage receive two cards.
