To start saving, take your Health Assessment to reach Bronze Vitality Status™ (if you haven't already). Sign in to HumanaVitality.com to take the Health Assessment. Then, request your Vitality HealthyFood Shopping Card right online.
Shop
Once you receive your Vitality HealthyFood Shopping Card, shop at Walmart for healthier foods that qualify as Great For You.
Save
Then, once your items are totaled, get your Vitality HealthyFood Shopping Card scanned at checkout. You'll save 5% on Great For You healthier foods to be redeemed on future purchases at Walmart**.
* HumanaVitality members must be 18 years of age or older to be eligible to participate in the Vitality HealthyFood program. The Vitality HealthyFood program is not available to all HumanaVitality members and is only available with certain plans or products offered by Humana.
** The 5% savings will be applied to that member's Vitality HealthyFood Shopping Card within 5-7 business days. Once applied, the savings may be used on any future purchase made at Walmart where gift cards are accepted.
