Bold Goal report

Humana’s Bold Goal is a population health strategy to help the communities Humana serves be 20% healthier by 2020. The Bold Goal focuses on improving key social determinants of health and chronic conditions through pilot programs and interventions with community and physician partners.

Bold Goal progress is tracked using the U.S. Centers for Disease Control and Prevention population health tool, “Healthy Days.” The tool takes into account the whole person by measuring both mentally and physically “Unhealthy Days” over a 30-day period, showing a direct link for Humana and its members between improved health, positive business results and social impact.

Value-based Care Report

Value-based care is different from the current fee-for-service model of care. It pays physicians for their services and also includes more pay for meeting quality measures, coordinating care, preventing repetitive treatments, controlling overall costs and improving health outcomes.

Humana’s Value-based Care Report details the company’s results for Humana Medicare Advantage members affiliated with healthcare providers in Humana value-based reimbursement models. On average, Humana’s MA members in value-based care arrangements experience healthier outcomes and improved quality of care.

Go365 3-year impact study

Productivity losses related to personal and family health problems cost U.S. employers $226 billion annually. Go365®, a wellness and rewards program subsidiary of Humana, helps employers and improves employee well-being. A 3-year impact study showed employees who were engaged in a wellness and rewards program had:

  • A 10% decline in health claims costs by year 3
  • Fewer unscheduled absences
  • Reduced lifestyle risk factors for chronic conditions
  • Fewer emergency room visits
