Humana Medicare Advantage plans

To help individuals get more out of their healthcare dollars, Humana offers Medicare Advantage plans that include benefits beyond Original Medicare (Parts A and B). With these plans, clients can take advantage of special features such as more predictable costs, which make it easier to budget monthly healthcare expenses and international coverage for medical emergencies.

Medicare Advantage plans in Minnesota

In Minnesota, Humana offers the following Medicare Advantage plans for individuals:

HumanaChoice® Preferred Provider Organization (PPO)

A PPO plan lets clients choose in-network or out-of-network providers. If they decide to go with network providers, their out-of-pocket costs may be lower.

Humana Gold Choice® private fee-for-service (PFFS)

This type of plan allows clients to see almost any provider as long as the provider is Medicare approved and accepts Medicare and the PFFS terms and conditions of payment.

Optional Supplemental Benefits

In Minnesota, Humana offers Optional Supplemental Benefits (OSBs) with our Medicare Advantage plans. OSBs vary by plan option and may include vision, dental, or other benefits.

Humana Medicare prescription drug plans

Humana’s stand-alone Medicare Part D prescription drug plans (PDPs) help individuals balance their health needs and healthcare costs. "Stand-alone” PDPs are separate from Medicare medical coverage. They let clients add prescription drug coverage to their Original Medicare (Parts A and B) coverage.

Medicare prescription drug plans in Minnesota

In Minnesota, Humana offers the following Medicare prescription drug plans for individuals:

Humana Walmart Rx Plan (PDP)

Low premium nationwide— $18.40 per month.

No deductible on Tiers 1 and 2, $360 annual deductible on Tiers 3, 4, and 5.

Plan offers copays as low as $1 on certain generic drugs at Walmart and Sam’s Club and mail-order copays as low as $0 for a 90-day supply of certain generic drugs at Humana Pharmacy®, the mail-order pharmacy that offers preferred cost-sharing.

Humana Preferred Rx Plan (PDP)

Prescription drug plan coverage with retail pharmacy copays as low as $1 for Tier 1 prescriptions when filled at pharmacies such as Walmart and Sam’s Club. $360 deductible.

Humana Enhanced Plan (PDP)

Humana’s broadest prescription drug plan coverage, with fixed copayments and no deductible.

Humana Medicare Supplement Insurance plans

Some individuals prefer Original Medicare plus supplemental insurance to cover additional medical expenses. Humana's wide range of Medicare Supplement — or "Medigap" — policies may be just what they’re looking for.

These plans help cover costs such as deductibles and coinsurance. Clients can keep the same doctors and are still in the Medicare program. They also have all the rights and protections associated with Medicare.

Medicare Supplement Insurance plans in Minnesota

In Minnesota,  Humana offers the following Medicare Supplement Insurance plans for individuals:

Basic

These policies offer basic benefits and cover Medicare Part A coinsurance for a skilled nursing facility. Clients also get discounts on some services such as foreign travel emergency care and outpatient mental health services. Optional riders are also available for purchase with this plan. Those riders include coverage for the Part A and B deductibles, Part B Excess charges (the difference in what the provider charges and what is accepted by Medicare), and preventive care (not covered by Medicare).

Extended Basic

Extended Basic plans include all the benefits of a Basic policy and additional coverage, including Part A and Part B deductibles, 100% Part B excess charges, and preventive care not covered by Medicare.

High-Deductible Coverage

The high-deductible option includes all of the basic coverage, like those offered in the Basic plan plus coverage of the Part A deductible, non-Medicare covered preventive care, and 100% of foreign travel emergency care. An annual, calendar year deductible must be met before benefits will begin.

50% Coverage

This plan offers basic coverage plus 50% coverage of the Medicare Part A deductible and hospital coinsurance, hospice care, skilled nursing facility coinsurance, and other services. There is an annual out-of-pocket maximum.

75% Coverage

This plan offers basic coverage plus 75% coverage of the Medicare Part A deductible and hospital coinsurance hospice care, skilled nursing facility coinsurance, and other services.There is an annual out-of-pocket maximum.

$20/$50 Copay Plan

This lower-premium policy offers basic benefits as well as coverage for skilled nursing care, Part A deductible, and 80% of travel-abroad coverage in emergencies. In addition to monthly premiums, there is an office copayment of up to $20 and an emergency room copayment of up to $50 (unless admitted).

Humana Pharmacy mail delivery shipments for new prescriptions are typically received within 7-10 days from the date of your order and in 5-7 days for a refill. If you don’t receive your shipment within this time frame, call 1-800-379-0092 (TTY: 711). Humana Pharmacy is available Monday – Friday, 8 a.m. – 11 p.m., and Saturday, 8 a.m. – 6:30 p.m., Eastern time.

Other pharmacies are available in our network.

The pharmacy network and/or provider network may change at any time. You will receive notice when necessary.

Humana MyOptionSM optional supplemental benefits (OSB) are only available to members of certain Humana Medicare Advantage (MA) plans. Members of Humana plans that offer OSBs may enroll in OSBs throughout the year. Benefits may change on January 1 each year.

You must continue to pay your Medicare Part B premium.

Humana’s pharmacy network, for both the Humana-Walmart and Humana-Preferred prescription drug plans, offers limited access to pharmacies with preferred cost sharing in urban areas of AL, CA, CT, DC, DE, GA, IA, IL, IN, KY, MA, MD, ME, MI, MN, MO, MS, MT, NC, ND, NH, NJ, NY, OH, OR, PA, RI, SC, SD, TN, VA, VT, WA, WV, WY; suburban areas of AZ, CA, CT, DE, HI, IL, IN, MA, MD, ME, MI, MN, MO, MT, ND, NH, NJ, NY, OH, OR, PA, PR, RI, VT, WA, WV; and rural areas of AK, DC, IA, MN, MT, ND, NE, SD, VT, WY. There are an extremely limited number of preferred cost share pharmacies in urban areas in the following states: CT, MA, MI, MO, MS, NC, NY, OH, RI, SC and VT. The lower costs advertised in our plan materials for these pharmacies may not be available at the pharmacy you use.
