Viewing Plans in Puerto Rico Change State

Humana Medicare Advantage plans

To help individuals get more out of their healthcare dollars, Humana offers Medicare Advantage plans that include benefits beyond Original Medicare (Parts A and B). With these plans, clients can take advantage of special features such as more predictable costs, which make it easier to budget monthly healthcare expenses and international coverage for medical emergencies.

Medicare Advantage plans in Puerto Rico

In Puerto Rico Humana offers the following Medicare Advantage plans for individuals:

Humana Gold Plus® Health Maintenance Organization (HMO)

These plans include the benefits of Original Medicare and more. They typically have low out-of-pocket costs but only cover care from network providers.

HumanaChoice® Preferred Provider Organization (PPO)

A PPO plan lets clients choose in-network or out-of-network providers. If they decide to go with network providers, their out-of-pocket costs will be lower.

Humana Medicare prescription drug plans

Humana’s stand-alone Medicare Part D prescription drug plans (PDPs) help individuals balance their health needs and healthcare costs. "Stand-alone” PDPs are separate from Medicare coverage. They let clients add prescription drug coverage to their Original Medicare (Parts A and B) coverage.

In Puerto Rico Humana offers the following Medicare prescription drug plans for individuals:

Humana Preferred Rx Plan (PDP)

Prescription drug plan coverage with retail pharmacy copays as low as $1 for Tier 1 prescriptions when filled at pharmacies such as Walmart and Sam’s Club.

Humana Enhanced Plan (PDP)

Humana’s broadest prescription drug plan coverage, with fixed copayments and no deductible.

Humana Medicare Supplement Insurance plans

Some individuals prefer Original Medicare plus supplemental insurance to cover additional medical expenses. Humana's wide range of Medicare Supplement — or "Medigap" — policies may be just what they’re looking for.

These plans help cover costs such as deductibles and coinsurance. Clients can keep the same doctors and are still in the Medicare program. They also have all the rights and protections associated with Medicare.

Medicare Supplement Insurance plans in Puerto Rico

In Puerto Rico Humana offers the following Medicare Supplement Insurance plans for individuals:

Plan A

Medicare Supplement Plan A is ideal for people who want more coverage than Medicare Parts A and B, but don't anticipate using a lot of healthcare services. It provides basic coverage for hospitalization, medical expenses, and hospice care. With Plan A, clients pay both Medicare Part A and Part B deductibles.

Plan B

Plan B provides basic benefits for medical expenses and hospice care. It offers a bit more than basic coverage for hospitalization. It may be a good fit for those who want more coverage than Medicare Parts A and B, especially for hospital expenses. Plan B enrollees are responsible for paying the Part B deductible.

Plan C

For clients who can pay a little more each month, Plan C offers predictable healthcare costs. Plan C includes more than basic benefits, including coverage for the Part A and Part B deductibles, skilled nursing care, and emergency care abroad.

Plan F

Plan F offers basic benefits plus coverage for both Part A and Part B deductibles, skilled nursing care, emergency care abroad, and Part B excess charges (the difference between what a provider charges and Medicare will pay). Plan F provides full coverage with few to no out-of-pocket expenses outside of monthly premium.

High Deductible Plan F

Plan F has a high deductible option offering the same benefits but requiring members to meet an annual, calendar year deductible before benefits begin.

Plan K

Relatively low premiums make these policies best for clients looking to control monthly costs while ensuring fair coverage. With a Plan K, 50% of claims are covered. To avoid higher expenses in cases of unforeseen medical need, Plan K offers clients an out-of-pocket maximum that limits what they have to pay annually.

Plan L

These low-premium plans offer a range of benefits including medical services, hospice care, hospitalization, and Part B coinsurance. The plan pays 75% of healthcare costs, and clients are responsible for the balance. There is an annual out-of-pocket maximum.

*Humana Pharmacy mail delivery shipments for new prescriptions are typically received within 7-10 days from the date of your order and in 5-7 days for a refill. If you don’t receive your shipment within this time frame, call 1-800-379-0092 (TTY: 711). Humana MyOptionSM optional supplemental benefits (OSB) are only available to members of certain Humana Medicare Advantage (MA) plans. Members of Humana plans that offer OSBs may enroll in OSBs throughout the year. Benefits may change on January 1 each year.

Enrollees must continue to pay the Medicare Part B premium, their Humana plan premium, and the OSB premium. Enrollees must use network providers for specific OSBs when stated in the Evidence of Coverage (EOC); otherwise, covered services may be received from non-network providers at a higher cost.

Humana’s pharmacy network, for both the Humana-Walmart and Humana-Preferred prescription drug plans, offers limited access to pharmacies with preferred cost sharing in urban areas of AL, CA, CT, DC, DE, GA, IA, IL, IN, KY, MA, MD, ME, MI, MN, MO, MS, MT, NC, ND, NH, NJ, NY, OH, OR, PA, RI, SC, SD, TN, VA, VT, WA, WV, WY; suburban areas of AZ, CA, CT, DE, HI, IL, IN, MA, MD, ME, MI, MN, MO, MT, ND, NH, NJ, NY, OH, OR, PA, PR, RI, VT, WA, WV; and rural areas of AK, DC, IA, MN, MT, ND, NE, SD, VT, WY. There are an extremely limited number of preferred cost share pharmacies in urban areas in the following states: CT, MA, MI, MO, MS, NC, NY, OH, RI, SC and VT. The lower costs advertised in our plan materials for these pharmacies may not be available at the pharmacy you use.
