  • Members must plan to return to the facility
  • You must pay your room and board and share of payments while away
  • You must live in the facility for at least 30 days between each departure
  • The facility must notify Humana Healthy Horizons in Florida of your departure within 24 hours

Assisted living facility move-in basket Get up to $50 worth of items once per lifetime

A move-in basket is available to members 21 and older who:

  • Live in an assisted living facility
  • Transition/move into an assisted living facility

Members can pick between two baskets:

  • Basket 1: An insulated tumbler, rainbow medication tray, and plush Sherpa home throw blanket inside a clear casual tote bag
  • Basket 2: A bottle holder, green carabiner, copper vacuum insulated bottle, domino set, and Arctic Zone thermal copper mug inside a black tote bag

Durable medical equipment/supplies
  • Members under care management for congestive heart failure can get 1 weight scale every three years
  • Members under care management for hypertension can get 1 digital blood pressure cuff every three years
Healthy living benefit Members can choose 2 healthy lifestyle aids, once per lifetime, to help maintain independence in their home:
  • 2 face mask
  • Clip on lamo
  • Digital scale
  • Home blood pressure cuff
  • Lumbar pillow
  • Peak flow meter
  • Personal fan
  • Reachers/grabbers
  • Walker bag
Hearing services
  • One hearing aid assessment, fitting, checking, and evaluation every two years
  • One in-ear monaural hearing aid per ear each year
  • One hearing aid, all other types, per ear every two years
Individual therapy sessions for caregivers Unlimited counseling visits for the people who help take care of our members Legal guardianship
  • Up to $500 per lifetime for members ages 17 and older in a skilled nursing facility (SNF) or private duty nursing (PDN) setting, and their parent is obtaining guardianship to protect individuals unable to care for their own well-being

Care manager approval required
Meal stipend (available for long-distance medical appointment day-trips) $200 per day (up to $1,000 per year) for same-day medical trips greater than 100 miles from home. Care manager approval required.

Nursing facility-to-community setting transition assistance $5,000 per lifetime to help a member move, for members who are:

  • Moving out of a nursing facility into their own home, and
  • Paying their own living costs

Outpatient hospital services The $1,500 fiscal year dollar limit for non-emergency outpatient services is waived. Pharmacy over-the-counter allowance $25 per household per month to spend on an approved list of products

Prevention kit – Flu/Pandemic prevention kit A flu/pandemic prevention kit that includes:

  • 10 3-ply face masks
  • Hand sanitizer
  • Oral digital thermometer


Available to first 1,000 Humana Healthy Horizons in Florida members (18 and older) who get a flu vaccine

We will send kits to qualifying members
Smartphone Coverage Through the Federal Lifeline Program, households may be eligible for 1 smartphone with monthly minutes, text and data.

Members who are under 18 will need a parent or guardian to sign up. Smartphone training for you and your caregiver during your case manager visits can be requested.

Members may also qualify for enhanced benefits through the Affordable Connectivity Program (ACP) that provides additional minutes, data and/or hotspot access.

Transportation assistance for caregivers For members living in an assisted living facility, eligible caregivers get four one-way trips per month to visit the member in the facility

To set up transportation, call member’s care manager
Transportation (non-medical) 3 round trips per month (within your local area) for members who live in a home- or community-based setting

  • One pair of frames* a year, or
  • A six-month supply of contact lenses with a doctor’s prescription

*Member pays any cost that exceeds $75 for luxury frames
