We have simplified COVID-19 care for our enrollees. See how we have you covered. Enrollees 18 and older can earn rewards for getting a COVID-19 vaccine.

Since the onset of the COVID-19 pandemic, we aimed to make your healthcare experience easier, more personal, and more caring. As many of you know, we’ve:
- Covered COVID-related costs
- Created a 24/7 call center
- Increased home services (e.g., in-home COVID-19 and diabetic testing, meal delivery, etc.)
- Sent masks and educational materials
- Supported our providers, so they can continue to offer the healthcare you need
- Waived your copayments
Get a COVID-19 vaccine and earn $40 in rewards
All enrollees 18 and older can qualify to earn $40 in rewards for getting a COVID-19 vaccine. To be eligible for this reward:
- Create an account by activating your online profile for Go365 for Humana Healthy Horizons™
- Get a COVID-19 vaccine
- If getting the Moderna vaccine, you must get both doses
- If getting the Pfizer vaccine, you must get both doses
- If getting the Johnson & Johnson vaccine, you must get one dose
- Log in to your Go365 for Humana Healthy Horizons account
- Upload an image of your completed vaccination card
- You must upload an image of your completed vaccination card within 90 days of getting your vaccine (either the second Moderna or Pfizer dose or the single Johnson & Johnson dose)
We will apply the $40 in rewards to your account after reviewing the photo of your vaccination card.
This reward is available to enrollees 18 and older once each year.
If you already received your COVID-19 vaccine, you can qualify to earn $40 in rewards, too. You have 90 days from June 1, 2021, to upload an image of your vaccination card. To be eligible, you must be:
- 18 and older
- A Humana Healthy Horizons in Kentucky enrollee at the time you received your COVID-19 vaccine
Rewards have no cash value. The monetary amounts listed above are reward values, not actual dollars. For some rewards, your doctor has to tell us that you completed the healthy activity. Once we get this information from your doctor, you will see in the app the option to redeem the reward. For any reward you qualify to earn during the 2021 plan year, we must get confirmation from your doctor by no later than March 15, 2022.
Go365 for Humana Healthy Horizons is available to all enrollees who meet the requirements of the program. Rewards are not used to direct the enrollee to select a certain provider. Rewards may take 90 to 180 days or greater to receive. Rewards are non-transferrable to other Managed Care Plans or other programs. Enrollees will lose access to the Go365® App to the earned incentives and rewards if they voluntarily dis-enroll from the Humana Healthy Horizons or lose Medicaid eligibility for more than one-hundred eighty (180) days. At the end of plan year (December 31), enrollees with continuous enrollment will have 90 days to redeem their reward. Rewards may be limited to once per year, per activity. We all know the quality of food we eat affects our health. For many people, an unhealthy diet is a common reality – a consequence of food insecurity.
According to the U.S. Department of Agriculture (USDA):
- Food insecurity is a household-level economic and social condition of limited or uncertain access to adequate food
According to Feeding America, more than 37 million people in the United States (or 1 in 9 Americans) live in food insecure households. Accessing healthy food, especially during the ongoing COVID-19 pandemic, has been difficult for many people. There are programs that can help.
Support for the communities we serve
Since the onset of the COVID-19 pandemic, we:
- Contributed $11.5 million for rebuilding, relief, equity, and inclusion efforts in Louisville, including:
- $6.5 million to help complete the Louisville Urban League’s Sports and Learning Complex
- More than $2 million to help the Louisville Urban League support the financial and employment development needs of low-wage essential workers
- Contributed $2.2 million for regional nonprofits, as part of our ongoing Community Partners Program
- Donated $550,000 to collaborative partnerships across the Commonwealth to help reduce COVID-19 transmission and treat people affected by it
- Donated between $20,000 and $45,000 to each of three Kentucky-area regional food banks to support food security in rural areas of the Commonwealth
- Made available up to 160,000 employee volunteer service hours to help revitalize downtown Louisville and communities across the country
