Tobacco & Vaping Cessation Coaching | 12 years of age and older | Tobacco & Vaping Cessation Coaching is focused on helping members with stopping their usage of nicotine products. Housing Support for SMI Members | 18 years of age and older | Members with Serious Mental Illness and eligible for Intensive Case Management who are homeless, at risk of becoming homeless, or transitioning from a behavioral health inpatient or residential facility without stable housing may be eligible for housing assistance up to $5,000. This once per lifetime assistance can cover:
